SmileSaver Dental and Vision Plan
The SmileSaver Dental and Vision Plan is available only in California. This renewable plan allows you to enroll yourself and your family for a minimum of three months. You should select a provider and enter the appropriate provider number(s) when enrolling online. Please Note: SmileSaver is a discount plan--not a dental insurance plan.
***IMPORTANT***
Those whose enrollment information and membership fee are received on or before the 20th of the month will have an effective date of the first day of the following month. Those who join after the 20th of the month will have an effective date of the first date of the second month thereafter. (Example: if your enrollment/payment is received on 9/20/07, your coverage will be effective on 10/01/07, but if your enrollment/payment is received on 9/21/07, your coverage will be effective on 11/01/07.)
Instructions For Locating a Dental or Vision Provider Online
When Searching for a Dental Provider
- Select "Dental HMO" and "California" and Submit
- Select "SmileSaver Plan 3000" and Submit
- Enter in your location information and Submit
When Searching for a Vision Provider
- Select "SmileSaver Vision HMO" and "California" and Submit
- Select "SM10" and Submit
- Enter in your location information and Submit
(Fac #) will be asked for later in the online enrollment process.
Where there is a double asterisk (**) in front of a provider name, please note that they are not accepting new patients.
