ART CENTER COLLEGE OF DESIGN STUDENT HEALTH INSURANCE PLAN

Term-Off Students and all eligible dependents of enrolled students may enroll online using a major credit card.

Please note that dependent enrollment is subject to verification of eligibility.  In addition, students enrolling a Domestic Partner in the plan must provide an Affidavit of Domestic Partnership to Renaissance Agencies, Inc. within one week of enrollment in the plan online.  Click here to download an affidavit.

USE YOUR INTERNATIONAL SOS TRAVEL ASSISTANCE SERVICES ON SCHOOL-SPONSORED TRIPS ABROAD
All students covered under this Student Health Insurance Plan have access to the International SOS Travel Services Program, while participating in any School-sponsored trip.  Please visit the Center for the Student Experience for more information. You may also access the SOS website directly.  Simply click here and enter the login # 11BSGC000068 where it says “Visit the Members Website “ on the left side of the page.

Deductible: $150 per plan year

Copay: $20 for most doctor visits, including emergencies

Treatment by a PPO provider/facility: 100%

Treatment by a non-PPO provider/facility: 70%

FILING YOUR REQUIRED CLAIM AFTER YOUR DOCTOR OR HOSPITAL VISIT

You must fill out a claim form or complete an online claim submission within 30 days of the date of visit to a doctor or hospital for treatment or services. Claim forms and online claim submission can be accessed below. Generally, you only need to submit one claim per condition. You do not need to submit a new claim for every follow-up visit, unless specifically requested.

FIND A DOCTOR OR HOSPITAL

Access to the First Health Network provides benefits nationwide for covered charges incurred at 100% of PPO charges when treated by a network provider. For a complete listing of PPO hospital and doctor facilities, click here.

Benefits are provided worldwide for covered charges incurred at 70% of actual charges when treated by non-network providers. Please note: treatment by a non-PPO anesthetist, radiologist, pathologist or emergency room Doctor in a PPO facility will be paid at the PPO level.

FIND A PHARMACY FOR PRESCRIPTION MEDICINES
Prescriptions are only covered under the plan when filled at an Express Scripts Pharmacy.  Click here to locate the one closest to you.

Students who reach the $1,000 plan year maximum for prescriptions should consult with their doctor about what programs are available for prescription assistance. Some examples are below.

PURCHASE OPTIONAL DENTAL/VISION DISCOUNT PLAN

General dental and vision coverage is not provided by the Student Health Insurance Plan. There is an optional dental/vision discount plan available to students and their dependents through SmileSaver for an additional cost.

PURCHASE SHORT TERM INSURANCE COVERAGE AFTER GRADUATION
To avoid a lapse in health insurance coverage, students should consider obtaining short term insurance between the time their coverage under the school plan terminates and permanent coverage is obtained through an employer or other source. If a lapse in coverage occurs, certain pre-existing conditions might not be covered under a new plan.  To see some short-term options that are available, click here.
