SMILESAVER DENTAL HMO AND VISION PLAN
SmileSaver is a pre-paid, managed dental plan available to you for a low monthly cost. SmileSaver is one of the largest providers of pre-paid dental plans in California and is available exclusively to California residents. You can select your own dentist from the SmileSaver provider directory, and there is no financial limit to the services you can receive. There are no deductibles, no claim forms, and no waiting for reimbursement.

MONTHLY MEMBERSHIP FEE
(You must purchase a minimum of THREE months)

Effective Date If online enrollment is completed (and payment submitted) on or before the 20th of the month, you will have an effective date of the first day of the following month. If online enrollment is completed (and payment submitted) after the 20th of the month, you will have an effective date of the first date of the second month thereafter. (Example: if your enrollment/payment is submitted on 9/20/11, your coverage will be effective on 10/1/11. But if your enrollment/payment is submitted on 9/21/11, your coverage will be effective on 11/1/11.)
Coverage for this plan does not extend beyond September 30, 2012.

You must select a provider and enter the appropriate "Facility ID" provider number(s) before enrolling online.
When Searching for a Dental Provider:

Under Plan Type, select "Managed Dental Plan"

When locating a provider please note the "Facility ID" is located below the provider name and dental office name. The provider code (Facility ID) will be asked for later in the online enrollment process.
Where there is a after the dental office name, please note that they are not accepting new patients.

TOTAL DENTAL ADMINISTRATORS DENTAL PLAN
The plan we offer for students in Arizona is Plan A800 through Total Dental Administrators. The TDA - A800 program is a comprehensive dental plan which makes dental care simple and easy to use. TDA has contracted with established private practicing dentists who provide you with convenient, affordable and quality dental care.
If online enrollment is completed on or before the 20th of the month, you will have an effective date of the first day of the following month.
