The deadline to enroll for the Spring Term is February 28, 2008.

Please note: students must have Full-Time Status, as determined by CIIS, in order to be eligible to enroll in this plan. Students have until January 29, 2008 (Add/Drop Deadline) to attain Full-Time Status through the school.

Please be aware that the Preferred Provider Organization (PPO) for this plan has changed.  Before seeking treatment, make sure that your doctor is a member of the new network or you may have to pay a higher coinsurance.  Click here to locate First Health Network providers.

Dental/Vision Discount Plan

The student health insurance plan does not include general dental or vision coverage.  If you would like to purchase a separate dental and vision plan, you may voluntarily enroll in the SmileSaver Discount Plan.  Please note this is a discount plan, not an insurance plan.

1.  Before enrolling in the SmileSaver Plan, you must select a dental provider for the Group Dental Plan 3000 and a vision provider for the Vision Plan SM10.  You will need the Code Number of the providers in order to enroll.

Please Note:
If you enroll online on or before the 20th of the month, you will have an effective date of the first day of the following month. If you enroll online after the 20th of the month, you will have an effective date of the first date of the second month thereafter. For example, if you enroll online on 9/20/07, your coverage will be effective on 10/1/07, but if you enroll online on 9/21/07, your coverage won’t be effective until 11/1/07.

Coverage for this plan does not extend beyond September 30, 2008. Please do not submit payment for enrollment in the plan beyond this date.
