HOW TO OBTAIN TREATMENT
  1. Always visit the Student Health Center before receiving any treatment! Obtain a Referral Form and keep it until you are ready to submit your claim.
  2. When choosing a doctor or hospital always try to use a Blue Cross Participating Provider—your coinsurance payment will be lower. Lists of Participating Providers are available at the Student Health Center or follow the instructions below to find a Participating Provider online.
  3. Call the doctor to schedule an appointment.

In an emergency situation go directly to the nearest hospital for treatment, or call 911 if you are unable to seek treatment yourself.


HOW TO SUBMIT A CLAIM

If you see a doctor or are treated in a hospital, there are two ways in which payment may be handled:



The provider may be willing to send the claim form directly to the company. In this case, the company will bill you for any charges that your insurance does not cover; or


The provider may require full payment at the time of treatment. In this case, you must do the following to be reimbursed for the allowable charges:

  • Send all Claim Forms, Referrals, Prescriptions, Laboratory Tests, and any other medical treatment information/bills to:

  • Keep copies of all your medical treatment information and documents for your records.
  • To check your claim status, call Blue Cross at (888) 850-4770.


You are responsible for submitting a claim in order to have your medical bills paid!

Make sure you answer every question on the claim form to ensure prompt processing of your claim!

Fall: 8/15/10 to 1/31/11

Winter: 12/15/10 to 1/31/11

Spring/Summer: 2/01/11 to 8/14/11

Summer: 5/15/11 to 8/14/11

Once you receive your ID card, go to www.gghstudents.com and click on “Health Insurance”. Sign in using the Certificate Number on your ID card to access information and tools regarding your plan.

 

Your ID card will be mailed to you by GGH or given to you by your advisor/administrator at school.

 

ID cards are mailed within 10-14 days of enrollment in the plan.  If you have not received your ID card yet and you need to access benefits or online information, please contact Renaissance Agencies, Inc. at 1-800-537-1777.


LOCATE A DOCTOR OR HOSPITAL

Select the following:

  1. Find a doctor—use Visitor Search if you have not received your ID card in the mail
  2. Select a Plan Type—select Large Group
  3. Select a Plan—select Blue Cross PPO (Prudent Buyer)
  4. Select a Provider Type and Specialty
  5. Click Next
  6. Choose “Enter Address” and type in the city/state OR zip code to find the doctors closest to you

STUDENT HEALTH CENTER

The colleges have student health centers on campus, which is a convenient healthcare option for students and scholars. The individual copayment will be waived on this insurance plan if you are first treated at the student health center. Please check with your college for the location and hours.
