Below you will find useful information about the 2010-2011 Lewis-Clark State College Student Health Insurance Plan (SHIP).

CHANGES TO SHIP FROM LAST YEAR

PPO NETWORK
Please be aware that this plan has incorporated into the coverage access to a Preferred Provider Organization (PPO) network of doctors, hospitals and other facilities who have contracted to provide specific medical care at a discounted, negotiated rate. Before seeking treatment, make sure that your doctor is a member of the new network or you may have to pay a higher coinsurance.

HOW TO WAIVE OUT OF SHIP
Students who are eligible to waive coverage under SHIP must sign onto WarriorWeb to submit a waiver request. The waiver must be completed by 5:00 p.m. on the Waiver Deadline Date each semester.

Deadline to Waive Coverage in SHIP
Fall: August 27, 2010
Spring/Summer: January 14, 2011

ENROLL DEPENDENTS IN SHIP

You may enroll your eligible dependents in SHIP online using a MasterCard or Visa.

Please note that dependent enrollment is subject to verification of eligibility. 

The deadline to enroll for dependent coverage is:
Fall:  September 14, 2010
Spring/Summer:  February 1, 2011

ENHANCED BENEFITS DISCOUNT CARD

Students (and dependents) participating in the Student Health Insurance Plan may enroll as a member in the Enhanced Benefits Discount Card Program. The Enhanced Benefits Card (EBC Card) is a multi-product discount savings program which allows members to receive various discounts on certain services and products when obtained through participating EBC Card network providers. It is not insurance. No claims are filed and reimbursements are NOT provided. Members must pay for all services and products themselves.
 
The following types of services are included:

• Vision (including exams, eyewear and LASIK)
• Dental (including cleanings, exams, X-rays and various procedures)
• Hearing (including hearing aids and other products for the hearing impaired)
• Prescription Drugs
• Diabetic supplies and monitoring
• Chiropractic treatment
• Lab tests and X-rays

To receive available discounts, members MUST use participating network providers and show their EBC Card at the time of service. To download the EBC Membership Card and locate participating providers, please click HERE to register. Please note an email address is required for registration.
