ID CARD
Your permanent ID card will be mailed to you after the start of Fall Semester. Permanent ID cards are mailed to you only once each school year (not each semester). If you need a replacement card, or haven’t received your card by mid-term, please contact AmeriBen at 1-888-955-1561.

If you need to access medical care prior to receiving your permanent ID card in the mail, click here to download a temporary card.

Always carry your insurance ID card with you.

ATTENTION!
As of September 15, 2012, Walgreens will begin to participate in the Express Scripts retail pharmacy network.

HOW TO WAIVE OUT OF SHIP
Please be advised that you must be registered for classes before you can submit a waiver application. Please allow at least 48 hours from the time of registration to submit your waiver application.

If you would like to waive out of the Lewis-Clark State College Student Health Insurance Plan for the Spring 2013 semester, waivers can be submitted from November 5, 2012 until January 28, 2013. Please CLICK HERE to submit a waiver application or check your waiver status during this time.

Students who are eligible to waive coverage under SHIP must submit a waiver application and all required documentation by the Waiver Deadline Date each semester.

Deadline to Waive Coverage in SHIP
Fall: August 31, 2012
Spring/Summer: January 28, 2013

Students who are covered under a health insurance plan that does not meet all of the applicable requirements will not be allowed to waive out of SHIP.

ENROLL DEPENDENTS IN SHIP

You may enroll your eligible dependents in SHIP online using a MasterCard or Visa.

Please note that dependent enrollment is subject to verification of eligibility.

The deadline to enroll for dependent coverage is:
Fall: September 15, 2012
Spring/Summer: February 1, 2013
